ACANY is a new care coordination organization providing community-based services and supports to people with I/DD. Our mission is to offer a more holistic, comprehensive, and person-centered level of service. Under this new state mandated healthcare model, Medicaid Service Coordination (MSC) will become an enhanced service called Health Home Care Management. Once ACANY begins operating as a CCO on July 1, 2018, we expect to provide services to more than 24,000 people with I/DD and their families across New York City, Long Island, and the Lower Hudson Valley.
